K-12 music educators in Kansas may receive one (1) college credit for attending the KMEA In-Service Workshop. The participating universities set the fee for this credit. Please complete and mail this page (page 1) directly to one of the participating colleges or universities listed on page 2. DO NOT mail this form to KMEA. A university representative will contact you. To receive credit, you must return the College Credit Verification form (page 3) to your participating university no later than April 1, 2012. Additional forms will not be mailed individually to teachers by KMEA. Please address all questions and concerns to your participating university.

This form must be postmarked by April 1, 2012 in order to receive ISW college credit.

This form must be mailed to the same university to which you mailed the College Credit Application form. The attendance policies to receive credit are as follows.

  1. Complete the information below for each clinic or concert you attended.
  2. Complete the "POINT" category as follows.
    1. Clinics or concerts scheduled for 60 minutes or less receive 1 (one) point.
    2. Clinics or concerts scheduled for over 60 minutes receive 2 (two) points.
    3. Mark 1 (one) point if you spent at least 45 minutes in the exhibit area or 2 (two) points if you spent at least 90 minutes in the exhibit area (maximum of 2 [two] points for exhibit time).
    4. To receive ISW college credit, you must have a minimum of 15 Points.
